REQUIRED DOCUMENTATION

Applicants should be prepared to provide the following documents at the time of application:

Proof of lawful presence: Driver's license, identification card, and/or passport

Social Security card

Income documentation, including as applicable:

Social Security income statement

Six most recent paystubs Current savings account statements

Unemployment statement

Self-employed 1099 tax documentation

Six most recent bank statements for checking accounts

Documentation for stocks, bonds, pension, 401(k), retirement funds, trusts, whole life insurance policies, and similar assets

If you have children, include:

Proof of lawful presence for the child: birth certificate and Social Security card

Proof of custody documentation

Custody and child support affidavit

Child support statement
